AFFIDAVIT IN SUPPORT OF APPLICATION

I, PERSON NAME of PERSONS ADDRESS, Homemaker, make oath and says as follows:

1. The facts and the matters contained herein are within my own personal information and knowledge except where otherwise stated to be on information, in which case I verily believe same to be true and correct. I am duly authorized to depose this affidavit in support of the application for declaration of paternity and change of names on the record on the entry of births.

2. I am the mother of the twin minor children PERSON NAME. At the time of their birth on the 16^th March, 2001 I was still legally married to one PERSON NAME (hereinafter “the said Badree”). In this regard, at the time of the registration of their birth, I stated that the father of the children was the said Badree as appears in their birth certificate hereto attached and marked “R.R.1”.

3. I knew that the father of my twin boys PERSON NAME was PERSON NAME (deceased) because I had ceased all martial and sexual relations with the said Badree since December, 1999 and was cohabiting with the said PERSON NAME (deceased). I then became pregnant for the said PERSON NAME (deceased) while cohabiting with him.

4. The said PERSON NAME e and I were finally divorced on 16^th April, 2003 and thereafter the said PERSON NAME (deceased) and I were married on 20^th January, 2006. A true copy of the marriage certificate is hereto attached and marked “’R.R.2”.

5. Since the birth of PERSON NAME, the said PERSON NAME (deceased) accepted and treated the children as his own. A true copy of a joint affidavit done by the said PERSON NAME (deceased) and I swearing to this fact is hereto attached and marked “R.3”.

6. The said PERSON NAME has always known and accepts that the said twins PERSON NAME are not his children. The said PERSONS NAME has acknowledged that, even though we were legally married at the time of the birth of PERSONS NAMES, he is not and could not be their father. He has also consented to this application for a Change of Name from “Badree”. A true copy of an affidavit of the said PERSONS NAME containing this consent is hereto attached and marked “R.R.4”.

7. My twin boys have known themselves to be possessed with the surname “SURNAME”, which has been their names registered in their respective schools and which they are also referred to, for all of their lives.

8. The said PERSON NAME do not possess a passport in their name “PERSON NAME” which is required for travelling.

9. I am a Guyanese national by birth and most of my family still reside in Guyana, inclusive of my father who is over the age of 80 years and who was recently ill. My twin boys have never left the country and I am desirous of taking them on a trip for the August school vacation to meet their family.

10. In these circumstances, I humbly pray that the reliefs sought in my application are granted herein, so that I can proceed with the passport application and travel plans.
